Dr. Mayatte is an In-Network provider with the following plans:
All other plans are considered Out-of-Network. We require time of service payment for plans that are out-of-network.
If you do not have naturopathic medicine insurance coverage, we offer a 20% prompt pay discount as well as pre-paid treatment packages to those that are under-insured or need to reach a deductible. (prompt pay means discount is offered if payment is made at the time of service).
We are happy to provide you paperwork to submit to your insurance company, allowing for possible Out-Of-Network reimbursement from your insurance company.
Labs are sent out to nearby laboratories who accept most insurances and bill directly to the patient.
We accept cash, check and credit/debit cards for payment. All services and supplies must be paid for in full at the time of your visit.
You may also use your FSA or HSA account to help pay for your appointments.
We cannot guarantee coverage of any services by your insurance. As a courtesy to you for out of network plans, we will provide you with a printed superbill for you to submit to your insurance for reimbursement of your visit. You will be responsible for full payment at the time of service. Please consult with your individual insurance policy to verify your out-of-network coverage, so that you are aware of your responsibilities. Insurance may not cover all fees, such as vitamin injections or outside labs that are considered investigational. We will do our best to make you aware of these non-covered fees ahead of time, so that you can choose if you'd like to go forward.
Fees are based on the length and complexity of the visit. Fees do not include additional procedures, lab tests, or supplements.
First Office visit*: $120-285
Return Office visit*: $80-225
In this initial visit, we will do a comprehensive health history and assessment. Knowing your healthy history is critical to determining underlying causes of illness. This generally takes 60 minutes. You will be asked to go online to complete your initial paperwork, consent to treatment and HIPAA notification forms.
Follow up appt is a 30 min appointment that will address treatment progress, new concerns and changes in treatment plans.
